Reviews Summary
Forma Restaurant & Cheese Bar is lauded for its innovative Italian cuisine, particularly the pasta dishes finished in cheese wheels and its extensive cheese selection. Diners praise the fresh, flavorful pasta and the restaurant's modern, chic ambiance. However, some reviews mention that certain dishes can be bland and that service can occasionally be slow or inattentive, though this is not a universal sentiment.
"Finally came and tried this place!
First the restaurant is really nice, there are some seats at the bar and the restaurant is quite big. And, they also have the spot where they prepare the dishes with cheese at the back of the restaurant.
In terms of options, there are a lot. We didn’t explore this time, but did see some cheeses that looked good. So, for my next time for sure. But, otherwise, there’s a variety of dishes options, which is great.
We got the burrata - that was the only thing a bit disappointing to be honest. Loved the seasoning of the beets. The leafs of salad were a bit blend. But, most importantly, the burrata itself wasn’t as tasty as I like it. In my opinion, it was not very tasteful, and I have definitely tried some way better than this one. I thought for a cheese bar they would have top burrata, but doesn’t look like it.
For the main dishes, we got very basic ones such as cacio e pepe and the bolognese. And, it was delicious. Their version of bolognese with the cream was so good. The pieces of cheese melting on top was perfect. The portion is also very decent.
They do have some dessert options, but we didn’t try.
Overall, it’s a great place with a very nice vibe. The food was great and I’m excited to come back and try more of it!"

"One of the best Italian spots I’ve tried in a while. The pasta was perfectly cooked, the ambiance was relaxed but elegant, and the service was genuinely kind. You can tell they care about every detail. Definitely coming back!"

"Went there for dinner and had a great experience. We made the reservation a few weeks in advance and sat inside. Really broad range of menu items and would highly recommend any of the items tossed in the cheese wheel. Very nice drink and appetizer selection as well."

Best Italian Restaurants in Los Angeles: Where to Eat Pasta & Pizza in LA
This perpetually lively eatery (which first opened on Santa Monica’s Montana Avenue) serves some of its pastas dalla forma—a preparation method in which the noodles are tossed in gigantic cheese wheels and arrive steaming, fragrant, and irresistibly cheesy. (Warning: The practice isn’t executed tableside, so if you want a peek, sneak over to the dining room’s back corner for a look-see.) For a simple, straightforward dish that truly showcases the perfection of Forma’s al dente noodles and the flavors of the cheese, ask for the Chitarra Cacio e Pepe—a combo of black pepper, extra-virgin olive oil, and slightly sharp, salty, melty Pecorino Romano. Don’t overlook the cheese bar either—there’s an overwhelming array of options, from pungent goat Cheddar to Caciotta al Tartufo (a nutty, sour, sweet, and truffle-flecked cheese made from cow’s milk). How to book: Reservations are available online.

Now with two locations on Montana Avenue and Venice’s Main Street, Forma is a Westside dining destination with two major reasons on the menu to visit: their beautifully plated cheese and charcuterie boards, and their pasta dishes tossed in giant wheels of Parmesan, pecorino romano and other types of hard Italian cheese. In the sea of Montana Avenue’s so-so chain restaurants and the countless mediocre Italian eateries dotting the Westside, Forma stands out as a go-to upscale neighborhood spot for a date, get-together with friends or any other special-ish occasion.
